This chart visibly off sync with the music. The notes should line up with the bottom of the waveform, not the top. 
The chart doesn't really do a good job of reflect the music. The chart is mostly single notes with the occasional random quad and nothing in between. A good chart should react to the music and follow the whole song, not just one instrument at a time. This doesn't mean you need to have a note for every sound, but you should pull from multiple elements when deciding what to follow.
